Terms and Conditions
Dr Danielle Kerley - Clinical Psychologist
Services Provided:
1. Dr Kerley provides Clinical Psychology Therapy Services for individuals, couples and families (clients) experiencing mental health or other psychological difficulties.
2. Sessions are provided by Dr Danielle Kerley.
3. Dr Kerley offers an initial consultation for 50 minutes and, on the basis of this assessment, then a further course of 50-minute sessions (of further assessment or intervention) may be recommended.
4. Initial brief assessment summary letters and end of therapy summary letters are included in the session fees if required/requested.
5. Reports can be written as required/requested.
6. Sessions are provided in appropriate clinic space or via online video platforms as appropriate/available. For online sessions, in the case of internet connectivity issues which do not immediately resolve, the session would revert to being over the telephone.
Dr Kerley’s Fees:
7. Clinical Psychology Sessions for Dr Kerley are £130 per hour.
8. Reports requested are charged at £130 per hour. If an extended report or a cognitive assessment is requested, the costs will be discussed with you.
9. If required, attendance at meetings or extended liaison work with professionals/family will be charged at £130 per hour or part thereof. If work will incur a cost this will be discussed and agreed with you in advance.
Travel costs and room hire costs:
10. Travel costs (to added locations – not regular clinics) if required are charged at the hourly rate of £130 per hour or part thereof.
11. There are no travel costs or room hire costs if sessions are held at the therapists’ clinic locations.
12. If an alternative clinic location is requested this may incur room hire costs. This will be discussed with you in advance.
Payment and Cancellations
13. Clients are responsible for ensuring prompt payment of fees.
14. Payments must be made via online bank transfer.
15. Payment should be made via online bank transfer 24 hours prior to the session to secure the appointment.
16. Sessions cancelled within 24 hours will be charged in full.
17. Sessions cancelled by Dr Kerley will not be charged and will be rescheduled at the earliest opportunity.
18. If sessions are to be paid for via a business/employer or company (excluding insurance companies and medico-legal work as listed below), then arrangements will be made in writing between the therapist and the funder as to invoicing arrangements. Sessions will typically be invoiced weekly or monthly as agreed and payment is expected within 7 days.
19. Payment can be made via online bank transfer payable to: Dr Danielle Kerley via BACS at account details: Sort Code 04-03-33 Account Number 56680097
Insurance Company and Medico-legal Work
20. Dr Kerley is registered with AXA and Aviva and can accept insurance-funded clients.
21. Insurance companies may require a referral via a GP. Please check your terms and conditions of your insurance provider as to what is required in advance of booking a session.
22. It is the client’s responsibility to ensure they are aware of any excess they need to pay on their policy and ensure this is paid promptly to their therapist.
23. If session fees will be covered via insurance cover, the therapist must be informed at time of session booking and the authorisation code and insurance membership number provided prior to the session.
24. If sessions are provided on the client’s information that this will be covered via their insurance company and this is not then settled by the insurance company, the client will be liable for the session fees.
25. If sessions are to be invoiced to a solicitor or third party due to ongoing litigation work this must be agreed in advance in writing and payment/invoicing details provided to Dr Kerley. Invoices will be produced on a monthly basis and payment requested within 7 days.
Please note acceptance of a session appointment or confirmation to proceed with sessions constitutes acceptance and agreement with these terms and conditions.
